RELEASE FORM FOR USING PATIENT’S OWN EYEGLASS FRAMES

We are pleased to accommodate the replacement of lenses in your personal eyeglass frame provided it is in good condition. Should we accept your frame for this service, we assure you that it will be handled with the utmost care. In certain instances, frames may exhibit signs of wear or brittleness due to age and usage, rendering them unsuitable to support new lenses. LIABILITY: If your frame incurs damage during the lens replacement process, the lenses intended for your specific frame may not be transferable to an alternative frame style. We are prepared to manufacture new lenses for any frame you provide or choose to purchase. The cost associated with acquiring any frame will be your responsibility. Upon completion of the service, we will return your damaged frame to you for examination. IF YOU HAVE READ THIS AGREEMENT AND WOULD LIKE US TO PROVIDE NEW LENSES IN YOUR FRAME: By signing and dating below, you acknowledge that you have read and understood our recommendations and policies regarding the use of your previously worn eyeglass frames. You still choose to have us fit your new prescription lenses into your own frame

How Lens Replacement Works

1. Pack Frame in Eyeglass Case:
Place your glasses in a sturdy eyeglass case to protect them during shipping.

2. Select Small Box or Bubble Mailer:
Choose a small shipping box or a padded bubble mailer to ship your frame.

3. Pack Your Frame Securely:
Insert your glasses into the shipping box or mailer. Use newspaper or packing foam to secure the frame and prevent it from moving around during transit.

4. Label and Seal the Box:
Tape the shipping label securely to the box and make sure it’s sealed properly. This will protect your frame during shipping.

5. Ship Your Frame:
Take your packaged frame to an authorized UPS or USPS location to mail it to PO Box 410205 Charlotte, NC 28241 

By following these steps, you’ll ensure that your frame arrives safely for lens replacement.

What type of prescription is required to order prescription glasses or contacts?

  • Your prescription must be current and unexpired, signed by a doctor.
  • It must include the brand name of the contact lenses you want to order if contacts.
  • It must be specifically for contact lenses (a glasses prescription won’t suffice).
